If you ask your doctor for a Better Health Plan, he/she will give you something that you personally are not committed too, and possibly don't understand. Try as you might that's unlikely to succeed.
On the other hand if you write your own Better Health Plan, including all the things you do and want to do, there's a fighting chance that you might be successful. Even if what you write isn't very well structured, it's a start and over time it can be improved.
This short online discussion will give you some bones, on which you can write your own plan.

Health Coaching is somewhat new as a professional discipline.
Health Coaches are NOT medical specialists, they have training giving them a wide view of health, rather than a deep knowledge. The objective of Health Coaching is to engage you in constructing a plan of action that will improve your own health.
The critical thing is that YOU need to develop your own plan to be more healthy.
You can only be successful if the plan is your own. No matter what your doctor, practice nurse or health coach might or might not know, the only effective tools you have, depend on what you know. So in the long run, we are looking to develop your own knowledge and health practices.
