There is a problem. The elephant in the room, must be named. You can talk about that. You and only YOU can take control of your own health.
There seems to be a problem immediately. I've tried to do that and I still have problems. For almost everyone, the first place to make change is in your diet. Nobody want's to imagine that their "healthy diet" isn't healthy. Sometimes (in fact often) the diet recommended by the Diabetes Association, isn't a healthy diet for diabetics. In my own case a "healthy runners diet" wasn't healthy for me.

The decline in our health that we refuse to see even when it's in plain sight. "Some conversations are forbidden."
The BIG intrusion in our lives, our poor health, is silenced. We are afraid. We hope. We pretend our poor health is "normal". We do nothing when we really need to tackle our health issues directly.
Good News: The idea the "normal health", being average health, is what you are looking for, needs to be opposed. The health of the general population is much poorer than desirable, and could be improved, if people had better knowledge.

This website has more than 600 pages of help, but the key is to understand that YOU and only YOU can do anything about this. Begin by knowing that our society is sick, but with good information and some effort YOU need not be sick.
Each day make an effort to learn more. There's lots of conflicting and confusing information, don't worry you will soon be able to sort out the good information from the rubbish.
Do some personal experiments as you go along.
This link allows you to download a two page PDF that will help you imagine a "Better Health Plan." Perfection is the enemy of progress, your first effort will be crude, but in six months time it will be very useful.
When you think you've got a good grounding, write a single page, Your Own Better Health Plan, and discuss it with your health advisers.
I asked Chat GP to give me an outline of a Better Health Plan. I got a well designed plan in 10 parts, with all the standard measures I'd expect. If you've got nothing else that would be good start. But it's generic, it includes many ideas once thought valid, that are today denied, or are questionable. Don't worry, if some of the ideas offered, are no longer valid, you'll soon identify them.
Writing a Better Health Plan that isn't perfect is still worth doing.
